How to get a driver’s license

A. STUDENT PERMIT TRANSACTIONS

1.  New Student Permit

•  Properly Accomplished Application for Driver’s License
•  Original Birth Certificate with Photocopy
–  NSO Authenticated Birth Certificate with Official Receipt
•  Medical Certificate issued by any licensed physician practicing in the Philippines

Qualifications:

1.  Must be at least 17 years old.
2.  Must be physically and mentally fit to operate motor vehicle.
3.  Must be able to read and write Filipino, English or the applicable local dialect.

Procedure:

1.  Proceed to the Customer Service Counter to get your checklist of requirements and secure a Driver’s License Application Form. (This form is available for download).
You can accomplish this form prior to transacting your business at the LTO. Get a queue number and wait for your number to be called.
2.  When your number is called, proceed to evaluator counter and submit all the required documents and have it checked for completeness and authenticity.
3.  Proceed to the Photo taking / Signature Area to have your picture and signature taken when your name is called.
4.  Proceed to the Cashier when your name is called to pay the necessary fees and obtain an Official Receipt.
5.  Proceed to the Realeasing Counter when your name is called and present the Official Receipt and claim the Student Driver’s Permit. You are required to write your name
and affix your signature in the appropriate Release Form.

Venue:     Any Licensing Center / Extension Office / District Office with DL Transactions

2.  Student Permit – Foreigners

•  Properly Accomplished Application for Driver’s License
•  Original Birth Certificate with Photocopy
–  NSO Authenticated Birth Certificate
•  Medical Certificate issued by any licensed physician practicing in the Philippines
•  Original and Photocopy of Passport
•  Alien Certificate of Registration I-card

Qualifications:

1.  Must be at least 18 years old.
2.  Must know how to read ans write in English or Filipino.
3.  Must be physically and mentally fit to operate a motor vehicle.
4.  Must have arrived in the Philippines for at least one (1) month prior to the date of application with LTO and can show evidence from BID/DFA that he will stay
in the country for at least one (1) year valid visa from the date of application.

Procedure:

1.  Proceed to the Customer Service Counter to get your checklist of requirements and secure a Driver’s License Application Form. (This form is available for download. You
can accomplish this form prior to transacting your business at the LTO). Get a queue number and wait for your number to be called.
2.  When your number is called, proceed to evaluator counter and submit all the required documents and have it checked for completeness and authenticity.
3.  Proceed to the Photo taking / Signature Area to have your picture and signature taken when your name is called.
4.  Proceed to the Cashier when your name is called to pay the necessary fees and obtain an Official Receipt.
5.  Proceed to the Releasing Counter when your name is called and present the Official Receipt and claim the Student Driver’s Permit. You are required to
write your name and affix your signature in the appropriate Release form.

B. NEW LICENSE TRANSACTIONS

1.  New Driver’s License – Non Professional

•  Properly Accomplished Application for Driver’s License
•  Medical Certificate issued by any licensed physician practicing in the Philippines
•  Valid Student Permit

Qualifications:

1.  Must be at least eighteen (18) years old.
2.  Must be physically and mentally fit to operate a motor vehicle.
3.  Must be a holder of a valid Student Permit (SP) issued at least thirty (30) days prior to the application.
4.  Must be able to read and write in Filipino or English.
5.  Must be clean, neat and presentable (wearing of sando, playing shorts and slippers during photo-taking and examinations will not be allowed).
6.  For Foreigners, the applicant must have been in the Philippines for at least one (1) month with the proof that he/she will stay in the country for at least one (1) year from
date of application.

Procedures:

1.  Proceed to the Customer Service Counter to get your checklist of requirements and secure a Driver’s License Application Form. (This form is available for download. You
can accomplish this from prior to transacting your business at the LTO). Get a queue number an wait for your number to be called.
2.  When your number is called, proceed to evaluator counter and submit all the required documents and have it checked for completeness and authenticity.
3.  Proceed to the Photo taking / Signature Area to have your picture and signature taken when your name is called.
4.  Proceed to the Cashier when your name is called to pay the application fee.
5.  Proceed to the Examination Room for the lecture and written exam.
6.  After passing the written examination, wait for your name to be called for the practical exam.
7.  After passing the practical examination, proceed to the Cashier when your name is called, present the Official Receipt and claim the Card type license. You are required
to write your name and affix your signature in the appropriate Released Form.

2.  New Driver’s License – Non Professional, Filipino w/ valid foreign license

5.  New Driver’s License – Non Professional, Foreigner w/ expired foreign license

6.  New Driver’s License – Professional

•  Properly Accomplished Application for Driver’s License
•  Medical Certificate issued by any licensed physician practicing in the Philippines
•  Valid SPR (Student Permit)
•  NBI
•  Police Clearance

Qualifications:

1.  Must be at least eighteen (18) years old.
2.  Must be physically and mentally fit to operate a motor vehicle.
3.  For RC 1, 2, 4 and 6 – must be a holder of valid Student Permit (SP) issued at least six (6) months prior to the application.
For RC 3, 5, 7 and 8 – must be a holder of valid Non-Professional Driver’s License.
4.  Must not be afflicted with any contagious disease or epilepsy.
5.  Must be able to read and write in Filipino or English.
6.  Must be clean, neat and presentable.
7.  Must not have been cited for two (2) or more counts of reckless driving during the period of validity of his/her existing license.
8.  For foreigners, the applicant must have been in the Philippines for at least one (1) month with proof that he/she will stay in the country for at least one (1) year from date
of applciation and is allowed to work herein.

Procedures:

1.  Proceed to the Customer Service Counter to get your checklist of requirements and secure a Driver’s License Application Form. (This form is available for download. You
can accomplish this form prior to transacting your business at the LTO). Get a queue number and wait for your number to be called.
2.  When your number is called, proceed to evaluator counter and submit all the required documents and have it checked for completeness and authenticity.
3.  Porceed to the Photo taking / Signature Area to have your picture and sugnature taken when your name is called.
4.  Proceed to the Cashier when your name is called to pay the application fee.
5.  Proceed to undergo written examination in the form of the PDL Basic Driving Theory Test for the RC/s applied for or Conductor’s License Examinations and will be
immediately informed of the results of the same.
6.  Execpt for the application for Conductor’s License, an applicant who passes the written examinstaion shall be required to immediately take the practical driving test for
the RC’s applied for.
After passing both examinations, the application shall be approved by the Approving Officer.

•  Applicants who fail to pass the Conductor’s License Examinations, Basic Theory Test and/or Practical Driving Test shall not be allowed to take the same test within a
period of one (1) month from the date of the last examination.
•  If the applicant fails to pass any of the written or practical examinations twice within a period of one (1) year, he/she shall not be allowed to apply for the same within a
period of one (1) year from the date of last examination.
•  If the applicant fails to pass any of the written or practical examinations thrice, he/she shall no longer be allowed to apply fo the same for a period of two (2) years
from the time of the lates failure.
•  In case the application is only for addtional RC/s, she/she shall be allowed to renew his/her PDL or Conductor’s License and, for this purpose, such shall be annotated
on the record of the applicant.

7.  After passing the practical examination, proceed to the Cashier when your name is called for payment of necessary fees, and obtain an Official Receipt.
8.  Proceed to the Releasing Counter, present the Official Receipt and claim the Card type License. Yor are required to write your name and affix your signature in the
appropriate Released Form.

7.  New Driver’s License – Professional, Filipino w/ valid foreign license

•  Properly Accomplished Application for Driver’s License
•  Medical Certificate issued by any licensed physician practicing in the Philippines

–  Translation from Embassy or Consular Officer
•  NBI
•  Police Clearance

 

8.  New Driver’s License – Professional, Filipino w/ expired foreign license

•  Properly Accomplished Application for Driver’s License

3.  Porceed to the Photo taking / Signature Area to have your picture and sugnature taken when your name is called.
4.  Proceed to the Cashier when your name is called to pay the application fee.

1.  Non-Professional Driver’s License Renewal

•  Properly Accomplished Application for Driver’s License
•  Medical Certificate issued by any licensed physician practicing in the Philippines
•  Current or Expired License

Procedures:

1.  Proceed to the Customer Service Counter to get your checklist of requirements and secure a Driver’s License Application Form. (This form is available for download. You
can accomplish this form prior to transacting your business at the LTO). Get a queue number and wait for your number to be called.
2.  When your number is called, proceed to evaluator counter and submit all the required documents and have it chekced for completeness and authenticity.

9.  Renewal of Driver’s License of Filipinos Living Abroad

Requirements:

1.  Photocopies of Passport (first page, visa page & departure in the Philippines and last arrival overseas).
2.  Letter of authority to authorize representative.
3.  Photocopy of Driver’s license and Driver’s license receipit (DLR), if available:
•  The Driver’s License shall be validated by issuing a driver’s license receipt (DLR) which will serve as temporary driver’s license (TDL).
•  The Licensee shal present himself/herself within thirty (30) days upon arrival to the same Officer where the renwal was made for photo-taking and submit the following:
♦  Driver’s License
♦ Medical Certificate issued by any licensed physician practicing in the Philippines
♦  Tax Identificatin No. / TIN Card (Photocopy)
♦  Photocopy of Passport

Procedures:

1.  Proceed to the Customer Service Counter to get your checklist of requirements and secure a Driver’s License Application Form. (This form is available for download. You
can accomplish this form prior to transacting your business at the LTO). Get a queue number and wait for your number to be called.
2.  When your number is called, proceed to evaluator counter and submit all the required documents and have it chekced for completeness and authenticity.
3.  Proceed to the Photo taking / Signature Area to have your picture and signature taken when your name is called.
4.  Proceed to the Cashier when your name is called to pay for the necessary fees and obtain an Official Receipt.
5.  Proceed to the Releasing Counter, present the Official Receipt and claim the Temporary or Card type License.

5.  Proceed to undergo written examination in the form of the PDL Basic Driving Theory Test for the RC/s applied for or Conductor’s License Examination and will be
immediately informed of the results of the same.
6.  Except for the application for Conductor’s License, an applicant who passes the written examinations shall be required to immediately take the practical driving test for
the RC’s applied for.
After passing both examinations, the application shall be approved by the Approving Officer.
♦  Applicants who fail to pass the Conductor’s License examinations, Basic Theory Test and/or Practical Driving Test shall not be allowed to take the same test within a
period of one (1) month from the date of the last examination.
♦  If the applicant fails to pass any of the written or practical examinations twice within a period of one (1) year, he/she sahll not be allowed to apply for the same within a
period of one (1) year from the date of the last examination.
♦  If the applicant fails to pass any of the written or practical examinations thrice, sh/she shall no longer be allowed to appl for the same for a period of two (2) years from
the time of the latest failure.
♦  In case the applciation is only for additional  RC/s, he/she shall be allowed to renew his/her PDL or Conductor’s License and, for this purpose, such shall be annotated
on the record of the applicant.
7.  After passing the practical examination, proceed to the Cashiers when your name is called for payment of necessary fees and obtain an Official Receipt.
8.  Proceed to the Releasing Counter, present the Official Receipt and claim the Card type License. You are required to write name and affix your signature in the
appropriate Released Form.

PENALTY FOR UNDERAGE DRIVERS

1.  A license driver found to have misrepresented his age but now of age and qualified to possess a driver’s license shall be required to surrender his previously issued license
for cancellation. However the same shall be restored to the licensee upon submission of a written appeal and payment of fine of Five Thousand Pesos (Php 5,000.00) plus
miscellaneous fee for revision of records.

1.1   The head of the District Office / Licensing Center shall forward to the License Section, Central Office the written appeal together with the official receipt of payment if
the misrepresentation was discovered and the licensee was cancelled by the Central Office.
1.2   The License Section upon receipt of the written appeal and Official receipt shall restore the cancelled driver’s license, by adjusting the first year of issuance of the
driver’s license to make it appear that the applicant was at least eighteen (18) years old when first issued a driver’s license.
1.3   If the misrepresentation was discovered at the agency level, the District Office / Licensing Center shall collect the corresponding fees and shall prepare and forward
Request for System Update (RSU), written appeal, Official Receipt (OR) of P5,000.00 to the License Section, Central Office to adjust the first year of issuance of the
license.
1.4   The license Section shall inform the requesting agency of the restored driver’s license for release to the licensee.

2.  If the Licensee opts for the one year cancellation of his/her driver’s license, he/she must surrender the Temporary Driver’s License to the head of the Licensing Center /
District Office and shall be barred from applying for a new driver’s license for a period of one (1) year reckoned from the time the Driver’s license / temporary Driver’s license
was surrendered.

2.1   The head of the District Office / License Center shall confiscate the Driver’s License / Temporary Driver’s license, prepares RSU informing License Section, Central
Office of the confiscated license and forward the physical documents to the License Section, Central Office within fifteen (15) days from the confiscation thereof.
2.2   The License Section shall place under alarm the cancelled driver’s license and the suspension shall be reckoned from the date confiscation was effected in the
District Office / Licensing Center of from the time License Section received the surrendered Driver’s License / Temporary Driver’s License in case where the District
Office / Licensing Center failed to comply with 2.1 hereof.
2.3   The licensee shall be allowed to apply for a student permit one (1) year after the cancellation of his/her driver’s license.
2.4   A licensee undergoin the one (1) year cancellation period of hi/her driver’s license has also the option to pay the fine of Five Thousand Pesos (P5,000.00) for the
immediate restoration of the license following the procedure for option 1.

3.  This shall also apply to the following:

3.1   Driver licensee who voluntarily request for correction of record due to age misrepresentation; and
3.2   Driver licensee whose license was cancelled due to underage prior to the promulgation of circular no. AHS-2008-1072

4.  This shall not apply to driver licensee found to be under age based on the year of issuance without misrepresentation of birthdate. The License Section upon receipt of the
written appeal and Official Receipt shall restore the cancelled driver’s license, by adjusting the first year of issuance of the driver’s license to make it appear that the
applicant was at least eighteen (18) years old when first issued a driver’s license.

It is the responsible of the Transportation District Officer to ascertain that documents submitted in support of an application for a driver’s license contain complete and accurate information as to age and identity of the applicant.
